He is survived by: his parents, Traci Perry (Ricky) and Michael Guziolek (Tammy); his sister Hailey; his stepsisters, Megan and Maddie; his brother Kyle; his grandparents, Jan Davis (Charles), Mike Petz (Jan), Vicki Musgrove and Rick Perry (Kathy); his great-grandparent Richard Perry; and his great-aunt Cheryl Redmond (George).
