He was predeceased by: his parents, Tony Czajka and Grace Czajka (Weigle); his wife Loula Belle Czajka (Price); and his daughter Terri Lynn Czajka. He is survived by: his children, Jerry Allen Czajka and Sherri Ann McCormick (Czajka); his granddaughter Alyssa McCormick; and his significant other Sue Sanchez.
