It is with great sadness that we announce the death of Jerry Prochazka (Toledo, Ohio), who passed away on March 18, 2023, at the age of 89, leaving to mourn family and friends. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

He is survived by: his wife Elizabeth; his children, Anna (Christopher) and Jerry (Jordan); his grandchildren, Cameron (Victoriana), Amber (Brandon), Justin (Layna) and Tiffaney; and his great grandchildren, Eden, Emrey and Kimber.

In place of flowers, the family would like to ask that donations be made in Jerry's name to a charity of your choice.
