He was predeceased by: his parents, Stanley Michael Kulakowski and Mary Jane Kulakowski (Szabo); and his in-laws, Mathew and Elizabeth Vedra. He is survived by: his wife Julia Ann; his children, David Michael Kulakowski (Tonya) and Debra Jo Diehlman (Dennis); his grandsons, Paul and George Kulakowski; and his siblings, Steven Kulakowski (Sharon), Susan Bojarski and Mary Kay Wing (Timothy).
