He was predeceased by: his parents, Max Lewandowski and Ann Kubiak; and his nephew Scott Kunkel. He is survived by: his girlfriend Nena; his children, Karen Vickerman (Jeff) and Brian Lewandowski (Heather); his grandchildren, Alexys, Kate, Zachary and Alec; and his sister Rosalie Shine.

The family requests donations in Phil's memory to The Toledo Humane Society, St. Jude's or any charity relating to dogs.
