He is survived by: his wife Debra Cook; his sons, William Cook (Amanda), Brandon Slomski (Barry) and Ryan Slomski (Mary); his grandchildren, Reed, Gabriel and Hope; and his siblings, James Cook (Judy), Suzanne Kaye (Chuck) and Scott Cook (Linda).
