She is survived by: her daughter Sherry Forman (Quinn) of Troy, OH; her siblings, Clarence Bramlette (Lisa), Ernie Bramlette (Teresa), Pete Bramlette and Sue Bramlette; her granddaughter Kendra Sue Green (Ethan) of Union, OH; her siblings, James Everett Forman (Brianna) of Troy, OH, Daniel Allen Forman (Mikayla) of Troy, OH, Sierra Maggert of Youngstown, OH and Lindsay Welker of Piqua, OH; and her great grandchildren, Anora, Emerald, Valor, Guinevere, Vivian, Noelle and Waylon. She is also survived by many nieces and nephews.
