He is survived by: his wife Kim Hayslett (Redmond); his children, Shane Hayslett (Brittany) of Troy, Brandy Kincaid (Marques) of Vandalia and Jason Brumbaugh (Jamie) of Troy; his siblings, Curt Hayslett (Sona), Terry Hayslett of Piqua and Karen Hayslett of Piqua; and his grandchildren, Korbyn Hayslett, Peyton Hayslett, Gianna Kincaid, Makai Kincaid, Karter Brumbaugh and Axel Brumbaugh.
