He was predeceased by : his parents, Walter B. Hook and Pauline Proffitt "Hook" Clark; and his spouse Shirley J. German. He is survived by : his children, Randy L. Hook (Cheri) of Van Wert, Tammy Evans (Brian) of Rockford and Todd A. Hook of Van Wert; his grandchildren, Sean Hook, Shane Hook, Matthew Hook (Amber) and Jamie Miller (Jason); and his sisters, Shirley Gossard of Cincinnati and Linda Amweg (Danny) of Ohio City. He is also survived by several great grandchildren.

PREFERRED MEMORIALS are to Good Shepherd Church of the Nazarene.
