He was predeceased by: his parents, Dr. John M. Duchak and Hope E. Duchak (Darden); and his wife Patricia Ann Duchak (Otto). He is survived by: his children, Joyce A. Duchak (Glenn Cline), David T. Duchak, Daniel J. Duchak and Kelli L. Duchak Kinnear (Keith); his stepdaughter Tamara A. Rawlins; and his sisters, Hope Pepe and Ruth Reinberg. He is also survived by nine grandchildren and six great grandchildren.
