He was predeceased by: his mother Alta Winifred Heakin Polivka (Burton). He is survived by: his father Roger Heakin of Cortland, OH; his sister Rhonda Orr (Kevin) of Gainesville, FL; and his stepsiblings, Dan Polivka, Andrew Polivka, Donna Polivka and Leslie Polivka Adams. He is also survived by many aunts, uncles and cousins.
