He is survived by: his former wife Deborah Angela Bokar; his children, Alex Mazzola, Samuel Mazzola III, Mark Mazzola, Frank Andrew Inman Jr., Seraphim Mazzola and Samantha Ann Inman; his grandchild Alexus Leighliter; his mother Susan Henderson; and his siblings, Russell Mazzola, Heather Mazzola, Steven Mazzola, Jennifer Babinchak (Robert) and Craig Henderson.
