He was predeceased by: his parents, Marion Cook and Nannie Adams Cook; and his sister Emma Jean Reed. He is survived by: his wife Denica A. Means Cook; his son Shade "Doug" Cook (Melanie Valentine); his granddaughters, Kristen Cook and Lacy Cook; his great grandchildren, Kaylee, John Wayne, Emma Rose, Connor, Scottie Jane, Braxton, Kinzlee and Sadie Grace; and his siblings, Roy Cook and Linda Climer. He is also survived by several nieces, nephews and friends.
