She was predeceased by: her parents, Melbourne Sullivan and Effie Fischer Sullivan; and her husband Lloyd Chalk (Butch). She is survived by: her siblings, Ivan, Ralph, Kenneth, Roy Lee, Jack, Glenn, Harold, Alyce, Gladys and Marvin Sullivan; her children, Kathy Chalk Gabbard, Kim Chalk Alfrey and Lloyd Chalk (Bud); her children-in-law, Craig Gabbard and Catalina Chalk; her grandchildren, Ariadna Jimenez, Jesus De Leon, Nathan Alfrey and Nick Alfrey; and her great grandchildren, Sophia, Alexandria and Betsy De Leon-Jimenez.
Visitation will be held on Monday, November 17th 2025 from 10:00 AM to 12:00 PM at the Hodapp Funeral Homes - West Chester (8815 Cincinnati Columbus Rd, West Chester Township, OH 45069). A funeral service will be held on Monday, November 17th 2025 at 12:00 PM at the same location.
