With heavy hearts, we announce the death of Sara Lyn Fields-Cook of West Union, Ohio, born in Georgetown, Ohio, who passed away on July 17, 2021 at the age of 37. Leave a sympathy message to the family on the memorial page of Sara Lyn Fields-Cook to pay them a last tribute.

She was predeceased by: her son Boston Bloomfield; her parents, Terry Fields and Velma Grooms-Kemmeter; and her grandparents, Willard, Linda Grooms and Sherwood "Woody" Fields.

She is survived by: her daughter Jaida Bloomfield of Seaman; her sister Erica Grooms (Shannon) of Peebles; her grandmother Louise Fields-Brown (Carl); her aunts and uncles, Nicki Knox (Jim), Stephanie Grooms (Zack Miller), Scott Grooms, Sonya Meyer (Marty), Betsy Jones (Richie), Steve Fields (Angie), Troy Fields (Cindy) and Scott Fields (Melissa); and her nieces and nephews, Courtney, Caitlin Grooms, Calvin and Corey Grooms. She is also survived by several cousins and friends.
