He is survived by: his mother Sara Floyd Slack of Wheelersburg; his wife Cindy Beck Slack; his sons, Michael Slack II (Amanda) of Wheelersburg, Jerrod Slack (Christina) of Wheelersburg and Shane D. Slack (Nicole) of Wheelersburg; and his siblings, Arva "Chip" Slack (Teri) of Columbus, Scott Slack of New Boston, Lolita Raynard of Sciotoville, Vernita Flannery (Mike) of Sciotoville, Carla Slack of Sciotoville and Randy Howard.
