She is survived by: her parents, Terry Sullivan and Linda Chestnut of Florence, KY.; her husband Shane Hensley of Williamsburg, OH; her children, Isabelle Middendorf, Logan Middendorf and Adam Middendorf; her stepdaughters, Emily Hensley and Lyla Hensley; her sibling Ryan Sullivan of Florence, KY.; her aunts, Norma Dwyer and Sherry Ramey; and her uncle David Tyree. She is also survived by several nieces and nephews.

Visitation will be held on Saturday, December 13th 2025 from 10:00 AM to 1:00 PM at the True Life Christian Church (15117 Eastwood Rd, Williamsburg, OH 45176). A celebration of life will be held on Saturday, December 13th 2025 from 1:00 PM to 2:00 PM at the same location.
