He is survived by: his sons, Stephen Kulyk (Tracy) of Midlothian, VA. and Jeffrey Kulyk (Sue); his granddaughters, Stephanie Kulyk and Michelle Kulyk; and his siblings, Rodney Kulyk of Cary, NC., Marsha Wilkins of Smyrna, DE. and Stacey Lanzarotta of Richmond, VA.

The family requested that material tributes take the form of donations to St. Charles Church, 7345 Westview Drive Boardman, OH, 44512, where Mike had previously served as an usher.
