He was predeceased by: his parents, Robert W. Mahaffey and Wilma Decker Mahaffey; and his sisters, Judy Jakubek, Donna McDaniel and Carol Goad.
He is survived by: his wife Sandra Lisko Liberto; his sons, Robert J. Mahaffey (Colleen) of Youngstown, Mark A. Mahaffey (April) of Hubbard and Brian A. Mahaffey (Charlene); his grandsons, Brian W. Mahaffey and Philip Liberto (Nicole); and his great grandchildren, Baylor and Brynlee Liberto.
