He is survived by: his wife Gayla Cook; his children, Michael Cook, II (Terri) and Cari Lyn Crisp (Monty); his stepchildren, Chris and Katie Callaway; his brothers, Claud Cook (Peggy) and James Hines (DeeDee); his sister-in-law Kathy Hines (Butch Cook); his grandchildren, Star, Skye, Misty, Kindra, Stori, Tara, Cody, Kennedy and Madison; and his great grandchildren, Indy, Wrigley, Ivy, Blake, Kindal, Addison, Emerson and Rami.
