He was predeceased by: his parents, Harvey Elmer Phillips and Ona Marie Phillips (Williams); his daughters, Sonya and Tonya Phillips; and his siblings, Judy Densmore and Donnie Phillips.
He is survived by: his wife Pamela Phillips; his children, Melissa Mullett (Jim Mullett) and Shawn Phillips (Kelly Phillips); his grandchildren, Johnathan Mullett, Anthony Mullett (Heather Mullett), Alex Phillips, Natili Phillips and Kylie Vanhorn; his great grandchild Lucas Mullett; and his siblings, Norman Phillips, Janice Moore, Patti Tyler (Terry Tyler) and David Phillips (Marsha Phillips).
