He was predeceased by: his parents, Dallas Dale McIntosh and Dorothy Lodema McIntosh (Wilkins); his sister Donna Dalene Weaver (McIntosh); and his nephew Danny Weaver. He is survived by: his wife Beverly Annette McIntosh (Wortz); his children, Brent McIntosh (Daphne), Brenda Jewell (Tim), Bonni Rott (Jim) and Dr. Brad McIntosh (Julie); and his grandchildren, Lurita Blank (Troy), Kelly Deigert (Scott), Nikki Dullum (Dan), Chris Jewell (Kristina), Kiyam McIntosh (Sarah), Olivia Obermark (Eric), Samuel McIntosh (Emily), Lillian McIntosh, Jaden Rott and Jared Rott. He is also survived by 9 Great Grandchildren.
