He was predeceased by : his parents, George L. Hayworth and Flonia I. Hayworth (Fillman); his siblings, Ruby Harrell, Viola Glover and Ronald Hayworth; and his great grandchild Jamey.

He is survived by : his family, Albert and John; his wife Sharon D. Bench; his children, Albert Hayworth (Anita), Terry Hayworth (Vickie), Ina Cody (Jestan), Michael Hayworth and John Michael Hayworth (Rachel); his grandchildren, Brook, Candy, Albert Jr., Justin, Sharon, Angell, Dani, Brandi, Bobby, Cody and Nicki; and his great grandchildren, Wyatt, Laney, Tru, Chi, Coulter of Bentley, T. J., Gus, Tensley, Stetson, Hagey, Luci, Katelynn, Kyle, Gina, Gracie, Madelynn, Morgan, Ethan, Travis, Jayce and Ashlynn. He is also survived by numerous nieces, nephews and extended family and friends.
