He was predeceased by: his parents, Bill Ballard and Norma Ballard (Gates); his siblings, Joe Ballard and Connie Schwartz; his parents-in-law, L. E. and Joan Castle; and his brother-in-law Jeff Miller.

He is survived by: his spouse Christie Castle; his grandchildren, Matt and Traci Ballard; his great grandchildren, Kizer, Keaton and Matthijs; his grandchildren, Michelle and Michael Sagely; his great grandchildren, Ashton Andrews of Claremore, Allison of Claremore, Emma of Claremore and Olivia Sagely of Claremore; his granddaughter Fanny Ballard; and his great grandchildren, Astrid, Henry and Björn of Örebro, Sweden. He is also survived by his wife.
