He is survived by: his wife Shirley Ann Cook; his daughters, Teri Matzdorf (Eric) and Lori Stovall (John); his grandchildren, Kyra, Kala Matzdorf, Betty Bohuslavicky (Beau) and Cole Hargus; his great grandchildren, Blakely Ann and Oliver Preston Bohuslavicky; and his sister Linda Green. He is also survived by numerous nieces and nephews.
