He is survived by: his wife Phillis Smith (Holderby); his daughters, Stefanie Johnson (Dave) of Decatur, AR and Crystal Shelley (Jeremy) of Colcord, Oklahoma.; his grandson Lance Osburn (Katsy); his great grandchildren, Ellie and Lincoln of Chickasha; his grandson Lane Osburn; his great grandchildren, Alinna and Riley of Siloam Springs, Arkansas.; his granddaughters, Lerami (Shelley) and Shawver (Morgan); his great grandchild Mason of Siloam Springs; his granddaughter Alyson Shelley (Jessie Martinez) of Colcord; his brothers, Ronny Smith (Melissa) of Colcord and Jack Smith (Pam) of Kansas; and his brother-in-law Jerry Holderby (Linda) of Tahlequah. He is also survived by nieces and nephews.
