He was predeceased by: his parents, Charles Wesley and Gladys Mae Doak; his wife Ann; and his siblings, Bob Doak and Nina Souleret.
He is survived by: his children, Lisa Ward, Steve Ward and Jerry Doak Jr.; his brothers, Gerald Doak and Mark Doak; and his grandchildren, Keri Romans, Lori Ward, Jerrod Ward and Jacob Tyler Doak. He is also survived by numerous great-grandchildren.
