He is survived by: his wife Kiley Collins; his children, Michael, Madison, Mason Collins and Jaycee; his mother Gayle Witham; and his brother Stanley Collins. He is also survived by several nieces and nephews whom he loved dearly; his aunts and multiple cousins; and his wife.
