She was predeceased by: her parents, Georgia Melba Lee and Carl Murphy; her children, Nelson "Randy" Cook, Sr. and Ashley Witkowski; and her brother Roger Murphy.

She is survived by: her husband Bradley Witkowski; her children, Gary Cook (Lindsay) of Savannah, MO., Nichole Hallford (Steven) of Duncan, OK. and Christina Judd; her brothers, Danny, Steve (Beverly) and Timothy Murphy; and her sister-in-law Pam Murphy of St. Joseph, MO. She is also survived by 10 grandchildren, 10 great grandchildren.
