He is survived by: his former wife Jean Smith; his children, Chris Johnson (Sheryl), Bryan Johnson, Christi Kuykendall (Greg) and Dawn Parker (Mark); his grandchildren, Jessica Alvarez, Kaci Ford (Alan), Kyle Kuykendall (Elizabeth), Ashley Hobbs, Justin Alvarez (Chloe), Austin Hobbs (Cherokee), Taver Parker (Shannon), Joshua Johnson (Sommer), Chase Parker, Skylar Parker and Wilmer Ayala; his siblings, Gary Johnson and Gloria Ryan; Cheri Alvarez.
Memorials may be made to Oklahoma Medical Research Foundation, Lupus/Rheumatoid Arthritis/Diabetes divisions with Ladusau-Evans as custodian of the funds.
