He was predeceased by : his mother Nancy McIntosh; and his brothers, Don and Mikey McIntosh. He is survived by : his father Roley McIntosh; his children, Cheyenne McIntosh of Glenpool, Austin McIntosh of Glenpool, John David Crabtree of Glenpool, Todd Crabtree of Houston and Rowdy Hendrix of Tulsa; his siblings, Marsha Daniels and Ronnie McIntosh of Oakhurst; and his grandson David Rhett. He is also survived by nieces, nephews.
