He is survived by: his wife Flora Lue Forrest; his children, Michelle Kay Harcrow of Siloam Springs, Arkansas and Bucky Ray Forrest of Siloam Springs, Arkansas; his grandchildren, Haleigh Sue Hensley (Jason) of Alma, Arkansas, Cordell Dean Haby (Destinie) of Afton, Hayzen Slack Phillips of Siloam Springs and Greyson Randal Harcrow of Siloam Springs, Arkansas; his great grandchildren, Oaklynn Dally Haby of Afton, Amelia Sue Hensley of Alma, Arkansas and Forrest Roper Dean Haby of Afton, Oklahoma; his siblings, Pat Daubs (Curt) of Warner, Oklahoma and Gerald Forrest (Sharon) of Fairland, Oklahoma; and his sister-in-law Sharon Forrest of Miami Oklahoma. He is also survived by several nieces and nephews.
