He was predeceased by: his parents, Ervin Edwin and Verna Cook (Stoner); his brother Lonnie Cook; his stepdaughter Marcia Perry; and his spouse Shirley Maxine Cook (Nash).

He is survived by: his children, Terry Cook (Cara) of Welch, OK, Rhonda Alston of Wagoner, OK and Jeannie Harlan (Don) of White Oak, OK; his stepdaughters, Missy Willard (Todd) of Miami, OK and Angie Asbell (Lou) of Springfield, MO; and his siblings, Roger Cook (Loretta) of Bluejacket, OK, Janet Porter of Bluejacket, OK, Joan Huddleson (David) of Bluejacket, OK and Sharon McGuire of Bluejacket, OK. He is also survived by 15 grandchildren and 18 great-grandchildren.
