He was predeceased by: his parents, Othel Cook Sr. and Lorraine Cook (Martin); his wife Bertha Cook; his siblings, Grady, Paul Cook and Linda Darden Cook; and his stepdaughter Gail Thacker.
He is survived by: his daughter Andrea Denise Cook Williams of Fort Smith; his stepchildren, Diann Spiros of Texarkana and Lee Garner of Muldrow; and his brothers, Othel Cook Jr. (Debbie) of Muldrow, Frank Cook (Patsy) of Muldrow and Laurel Cook (Deborah) of Muldrow. He is also survived by several grandchildren, great-grandchildren, nieces, nephews and cousins.
