He was predeceased by : his parents, Zearl Lewis and Violet Hetler Lewis; and his brothers, Carlus Lewis, Edwin "Sonny" Lewis and Charles Lewis.

He is survived by : his wife Alice Louise Little; his children, Rainy Lewis Garner (Aaron) of Hulbert, Allyssa Toyebo (Steven) of Muskogee, Kayley Lewis of Muskogee, Joel Lewis (Jenna) of Muskogee and Lacey Wolfleg (Brandon) of Muskogee; his grandchildren, Breahna Garner, Abigail Garner, Paisley Rose Toyebo, Kellan Toyebo, Jensen Toyebo, Callum "Ellis" Lewis, Micah Lewis, Malakai Lewis, Aubree Hubbard, Ashlynn Hubbard, Everleigh Wolfleg, Scarlette Wolfleg and Adaline Wolfleg; and his brother Billy Ray Lewis of Midwest City. He is also survived by numerous nieces, nephews, other family members and friends.

“I are so sorry to hear of Dales passing. Dale was a special man who loved and cherished his family. I wish I had the right words; just know I will be in prayer for all of you as you navigate through the coming days. I remember when Jeremy was little, we asked him who wrote the Sermon on the Mount...he didn't miss a beat, he said it was Dale that wrote them. Even he saw that Dale was close to God. Cherish the memories of him and let them help you through your grief.”
