He is survived by: his son Jeremy Blake; his mother Pamela Blankenship; his brothers, Jeff (Hope), Jason Blake (Angie) and Travis Shaw (Noreen); his nieces and nephews, Cassidy, Alex, Avery, Arden Blake, Nyla, Arya Shaw, Addison, Mikayla and Johnna Kelley; his aunts and uncles, John Blake (Nadine), Tom Blake, Ann Blake, Regina Olson (Ron), Deborah Harwood (John), Barry Grife (Cindy) and Lori Schlichter (Todd); and his sister-in-law Sheila Johnston (Steve).
