He was predeceased by: his parents, George Hayes and Ora Hayes (Bowers). He is survived by: his children, Shawn David Hayes (Beth), Patrick Taylor Hayes and Dr. Sarah Marie Sanchez (Hayes); his niece Amber Kleman; his wife Julianne (Burt); his great-nieces and great-nephews, Noah, Orie and David (Sarah); his siblings, Randy (Alice) and Tina; Patrick.

David has requested in lieu of flowers, all memorial contributions may be made to The Refuge and Meadowcreek United Methodist Church, both in Collinsville, Oklahoma.
