He was predeceased by: his mother LaVeda Woolfolk. He is survived by: his father Orvid Delane "Skip"; his wife Carla Woolfolk of Piedmont Ok; his children, Casey, Megan and Lora; and his sisters, LaNell Wagnon and Michelle Martin. He is also survived by six grandchildren, and numerous nieces and nephews.
In lieu of flowers, the family asks that donations be made to * Great Plains Diabetes to support their Camp Live Well summer camp for children with diabetes.
