"Sonny" Cook; his wife Gerrie Lynn Cook of Oklahoma City, OK; his children, Blake Wayne Cook of Ponca City, OK, Angela Marie Cook of Stillwater, OK and Justin Lee Cook of Ponca City, OK; his brother David Lee Cook (Diana) of Globe, AZ; and his aunts, Christi Leatherman of Grapevine, TX, Tammy Stewart of Bartlesville, OK and Kay Duroy (Chris) of Shawnee, OK..
