He was loved and cherished by many people including: his parents, Phillip Kozak and Imogene Kozak; his siblings, James of Tulsa and Kathleen Hooten (Carlin); his siblings' children, Jeff Carpenter of Tulsa, OK and Jayna Self of Nixa, MO; his great-nephews, Jewell, Andrew and Hunter; and his great-niece Hannah.
A funeral Mass will be held on Wednesday, December 10th 2025 at 1:30 PM at the Holy Family Cathedral (820 S Boulder Ave, Tulsa, OK 74119).
