He is survived by: his wife Kathleen Ann "Kathie" Bolick; his children, Michelle Renee Bolden (Steve Bolden) and Michael Douglas Bolick II (Keesha Bolick); his grandchildren, MacKenzie Michelle Bolick, Zachary Ray Bolden, Mallory Madison Bolick, Sophia Renee Bolden and Michael Douglas Bolick III; and his brother Mark Bolick (Vicki Bolick).
