He is survived by: his children, Trey Ballew (Dawn) of Tulsa, OK and Katy Ballew of Tulsa, OK; his siblings, John Ballew (Lori) of Pryor, OK and Linda Kay Sylva of Washington; his sister-in-law Pat Cantey of Pryor, OK.; his grandchildren, Ashlee Sky Ballew, Jaiyana Simmons, Aariyana Simmons, Kayiana Simmons and Cayden Simmons; the mother of his children Carlene Ballew; and his girlfriend Misty Goad.
