He is survived by: his daughter Dana Lee Cook (Greg) of Wewoka; his wife Marlene Crittenden (Leewright); his brother Mickey Crittenden (Linda) of Edmond, Oklahoma; his grandchildren, Jordan, Stephanie Cook of Shawnee, Austin and Kristi Cook of Seminole, Oklahoma; and his great grandchildren, Carter Cook, Canaan Cook, Cason Cook, Zane Cook, Everly Cook and Macrae Cook.
