He was predeceased by: his parents, David I. Collins and Gertrude Hull Collins. He is survived by: his wife Mary Jo; his daughters, Lisa Stumph (Mike) and Michele Collins (Dave); and his granddaughter Jordan Crane. He is also survived by 4 sisters and 1 brother, and numerous nieces and nephews.
In lieu of flowers donations may be made in Dale's memory to Evergreen Hospice House.
