He was predeceased by: his parents, Bula Smith White (Pendleton) and Herald Wellington White Sr. (Cottage Grove); and his daughter Vicki Booth. He is survived by: his wife Phyllis Johnson White; and his children, Laurie Perrett, Nancy McCloud, Molly Benge, Lori Jackson and Teri Fowler.
