J. Hazelett; his great grandchildren, Brooklynn Hazelett and Gage Hazelett; and his siblings, Stafford (Nancy Hazelett), Karsten (Josephine Hazelett), Alysa (Lawrence Hilton), Tabitha (Patrick Becker), Renee (Jeffrey Smith) and Cheryl Akeman. He is also survived by his five grandchildren and their spouses.
