He was predeceased by: his parents, Donald Jack Slack and Leona Slack. He is survived by: his wife Jane Packard; his children, Craig (Debrah) of Meridian, ID, Keith (Stephanie) of Seattle, WA and Lynne (Rich) of Monroe, WA; his grandsons, Erik (Jannalyn) and Alex; and his brother Malcolm (Nancy).
