He is survived by: his wife Nancy Forth of Pendleton; his children, Mike Forth of Pendleton, Joe Forth (Ronald Venancio) of Medford, MA, Chris Forth of Pendleton, Cindy Forth of Curtice, OH and Stacy Sewell (Steve) of Burlington, WA; and his grandsons, Brendan Sewell, Nathan Sewell (Natasha) and Jeff Bodewig.
