He was predeceased by: his parents, Raymond Anthony and Naomi Blazak (Wickline); and his sister Kathy Goodwald. He is survived by: his wife Bonnie G. Blazak (Holzer); his son Raymond K. Blazak; his sisters, Conny Brown (Lenny), Brenda Black (Jerry), Roberta Fleet (Joe) and Dianna Shrum (Mike); his grandchildren, Kayla Angleton (Eric) and Anthony Blazak (Tammy Gaydos); and his great grandchildren, Abel and Owen. He is also survived by numerous nieces, nephews, and friends.
