She was predeceased by: her parents, Robert Teramoto and Gail Teramoto; her son Jeremy Teramoto-Ursida; and her grandparents, Edward and Tatsuko Matsumoto.
She is survived by: her husband David Ursida; her children, Malia Ursida-Cain (John Michael) and Angelo Ursida; and her aunts and uncles, Donnie, Floyd, Miles Matsumoto, Faye Kikuchi (Douglas), Deborah Matsumoto, Reinton Kawamae and Jeph Miyaji. She is also survived by two brothers and one sister.
Memorial donations may be made to Jude's Children Research Hospital.
